Lifting Voices - Parent or Caregiver Survey

 

This survey is designed for parents or caregivers of children of any age, with a mental health and/or substance use condition.

We will use this survey to lift up your voice to help other parents and caregivers and their children. We will share what we learn from this survey with the people who are in charge of overseeing and funding mental health and substance use services, but your individual response will remain anonymous.

We will NOT disclose any information that would identify you or your family.  

 

Guidance:

 

Please complete only one survey per family for one child who has required intervention for any of the following conditions: learning, attentional, behavioral, mental health, and substance use.

Please complete the entire survey, but feel free to skip.

We estimate that this survey will take you about 15 - 30 minutes.
