We invite you to apply for Integrated Care DC’s yearlong Value-Based Care Learning Collaborative. This application can be completed on your own or with your team. Your responses will be used to enroll you in one of three content-specific tracks where you can deep-dive into improving a value-based care (VBC) metric tied to better care and outcomes for those you serve.

- I understand this is a participatory learning collaborative, over the course of a full year, including approximately monthly engagement opportunities.
- I will attend in-person and virtual sessions to the extent possible.

- I will be an active participant in discussions and peer sharing.
- I will complete the quality improvement tasks associated with this learning collaborative that are necessary to improve my VBC outcomes, including a logic model, PDSA cycles, data reporting, and barrier mitigation and sustainability activities.
Successful participation requires a dedicated team with cross-organizational buy-in. At least one person should be identified for each role of the following roles (the same person can fulfill more than one role if they have the qualifications to lead both roles):

- QI Lead
- Clinical Lead
- Data/Informatics Lead
